HIGH INTENSITY DAILY PERSONAL ACTIVITIES

  • Complex Bowel Care includes:

    • Observing and recording changes in a patient’s bowel habits;

    • Administering treatments such as enemas and suppositories;

    • Identifying when the patient needs to be referred to a practitioner;

    • Administering non-routine medication; and understanding that bowel health is highly personal for the patient and supporting them accordingly.

    • Stoma Care

  • Urinary Catheter Management - Care provided to participants who are unable to urinate on their own or are immobile

  • Subcutaneous injections administration - includes insulin, opioids, heparin, epinephrine, and allergy medication.

  • Complex & Simple Wound Management - includes post-surgical wounds, pressure injuries and chronic wounds such as diabetic ulcers, venous leg, suture & staple removal and infectious wounds.

    • Referral to a wound specialist.

  • Medication Management

    • Overseeing the medications prescribed to participants.

    • Safe and timely administration of medications in accordance to participant’s needs, goals, and preferences to optimize health and wellbeing.

COMMUNITY NURSING

  • Provision of care, training and supervision to respond to the complex care needs of a participant where that care is not the usual responsibility of the health system;

  • Continence aids: assessment, recommendation, training delivered by a nurse.

DAILY LIVING SKILLS

  • Support and care assistance provided to promote general life skills to increase independent living. These include:

    • Assistance in entering agreements; maintaining records; caliming payments from the NDIA; and paying providers;

    • Public transport training and support;

    • Developing skills for community, social and recreational participationl

    • Showering, toilet training and personal care;

    • Shopping for groceries and planning meals;

    • Perform household tasks such as clothes-washing and vacuuming;

    • Develop problem-solving skills.

DAILY PERSONAL ACTIVITIES

  • Assistance with, or supervision of, personal tasks of daily living;

  • Support worker can live in the house of, or travel with the participant;

  • Assistance with, and/or supervision of, personal tasks of daily life to develop skills of the participant to live as autonomously as possible;

  • Assistance with self-care activities during the day or evening;

  • Support to enable participants to engage in social or recreational activities.

HOUSEHOLD TASKS

  • Performing essential house and/or yard activities that the participant is not able to undertake:

    • Garden Maintenance

    • Cleaning

    • Drying

    • Washing

    • Vacuuming

    • Grocery shopping

  • Preparation and delivery of meals to a participant who is unable to do this themselves.

PARTICIPATE COMMUNITY

  • Provision of support to enable a participant to engage in community, social and/or recreational activities including but not limited to:

    • Seeing a movie or a concert;

    • Attending personal development courses;

    • Going to community events;

    • Joining a social group or a sporting club;

    • Going on a holiday camp;

    • Visiting your local library;

    • Taking an outing or a holiday.
